7.6 Team Qualities Case Application

Team Success Strategies:

Dr. Patel (Primary Physician): Dr. Patel consults with Chris (physical therapist) and Emily (speech therapist) before making any changes to Mary’s medication that might affect her therapy sessions. This demonstrates collaboration and respect for other team members’ expertise. Actively participates in scheduled interdisciplinary team meetings to discuss Mary’s progress and adjust her care plan.

Chris (Physical Therapist) and Emily (Speech Therapist): Chris and Emily coordinate their therapy schedules to maximize Mary’s participation and avoid conflicting sessions and fatigue. This shows effective teamwork and prioritization of client-centered care. Engage in interdisciplinary team meetings, contributing to discussions and adjustments in Mary’s care plan.

Individuals Lacking Demonstration of Team Strategies:

Laura (Social Worker): Struggles to include Mary’s family in discharge planning discussions but moves forward with client placement paperwork independently. How to Modify Approach: Make a concerted effort to involve Mary’s family in discharge planning discussions, perhaps by scheduling family meetings or using video conferencing if the family cannot be present. Actively seek feedback from Mary and her family regarding their preferences and needs for discharge planning to ensure the chosen facility is acceptable to all parties. Share concerns and discuss discharge plans in interdisciplinary meetings to gather input and support from other team members.

Lisa (Dietitian): Identifies a dietary plan that she believes will best meet Mary’s nutritional needs but disregards Mary’s dietary preferences. How to Modify Approach: Work to incorporate Mary’s dietary preferences into the nutritional plan to ensure it is both effective and acceptable to Mary. Engage in discussions with Mary and her family about dietary preferences and restrictions, explaining the importance of certain nutritional choices while finding a balance that meets Mary’s needs and preferences. Discuss the dietary plan and any challenges in interdisciplinary meetings to get insights and support from other team members who may offer additional strategies for addressing dietary concerns.
